1. This offer will be valid only for Sampath Mastercard World Credit Cardholders.

2. In order to enjoy the promotion, the full payment needs to be settled with a Mastercard World Credit Card.

3. 20% discount applies exclusively to aesthetic, dental, and plastic surgery treatments.

4. Consultation fees, medications, lab investigations, and post-operative care items are excluded from this offer.

5. Appointment Requirement: Prior appointment is mandatory.

6. Offer is non-transferable and applicable only to the cardholder and the family.

7. Each cardholder may use the discount once per treatment category (aesthetic/dental/plastic surgery) during the campaign period.

8. This offer cannot be combined with any other ongoing promotions, loyalty rewards, or discounts.

9. Cardholders must present valid identification and the eligible Mastercard World Card at the time of payment.

10. Maya Clinic reserves the right to refuse the discount if terms are not met or if misuse is suspected.

11. Treatments are subject to availability of practitioners and appointment slots.

12. Offers cannot be combined or clubbed with any other ongoing promotions. Each offer stands independently and is subject to its specific terms and conditions.Partner and any claim by the Cardholder against the Partner will not relieve the Cardholder from his/her obligation or repayment to the Bank.

13. The Bank accepts no responsibility in any way for any goods/services purchased and any dispute relating to same should be resolved by the Cardholder directly with the Partner and any claim by the Cardholder against the Partner will not relieve the Cardholder from his/her obligation or repayment to the Bank.

14. Mastercard shall assume no responsibility for the quality of service/products provided.

15. Mastercard reserves the right to withdraw, modify or change all or any of the rules, terms and conditions applicable to this offer at any given time without prior notice to Cardholders.

16. The Bank reserves the right to modify or change the Terms & Conditions of the offer.

17. The general Terms & Conditions of Sampath Bank and Masterdard shall apply.
