 Required Documents

1. Duly filled Corporate Card application with the personal details of individual nominated from the Company
2. Certified copy of the NIC / Passport / Driving license
3. Certified copy of billing proof (If permanent address in application differs from the address stated in the NIC or Driving license)
4. Duly filled KYC forms of applicants and company Directors
5. Declaration of beneficial ownership should complete with Directors details and signed by Directors with Company seal
6. Board Resolution authorizing to obtain Credit cards, printed on the company letter head along with the applicant's name NIC number and requested credit limit(Extent).
7. Form of Indemnity (standard format) printed on the company letter head signed by Two Directors or a Director with company secretary according to company articles.
8. Certified copy of Certificate of Incorporation/Business Registration
9. Certified copy of List of Directors ( Form 01 / 20 or 40 )
10. Certified copy of Articles of Association
11. Last year Financial account statements ( P&L account and Balance sheet ) certified from the company
12. Certified copies of company current account statements of last six months (If other bank accounts are maintaining)
